Unfortunately, we are not in-network with ANY Medicaid, traditional Medicare, or Medicare Advantage plans at this time, including those managed through companies like Anthem.

We utilize Headway for our insurance billing and payment services; new patients will setup an account with Headway during the intake process. Any questions or concerns regarding insurance and charges can be directed to Headway.

 

We are accepting other patients on an out-of-network/cash pay basis at this time and can provide you with superbills to submit to your insurance company for reimbursement. Fees are currently as follows:

• Initial evaluation: $250 (60 minutes, varies with complexity)

• Follow-up appointments: $100-$250 (varies with time, complexity, services included)

• Intakes: 99205 (60 minutes)

• Virtual care (beyond 5 minutes, via patient portal): 99421, 99422, 99423

 

Fees that may apply but are not generally reimbursable via insurance:​

• Refills outside of appointment: $25

• Form completion outside of appointment: $25 per 15 minutes

• Late cancellations (within 48 hours)/no-show: $100-$150 for follow-up, $200 for intake

• QBCheck: $75 per test (typically 2 tests required, may be covered by insurance)
